• A copy of constitution of the Association and Registration documents from the proper authority attested by the association.
• List of Board Members on Executive Committee members or
• Copy of Board Resolution of the Association (guideline has to be mentioned in the resolution for opening BO Account, authorized persons to operate the account)
• Two copies of recent colored photographs of the each of the BO Account operator with name and signature on the back of the same.
• Photocopy of National ID of each of the Account Operators with signature as per National ID.
